What is an EAP?
Welcome to our Matrix Employee Assistance Program (EAP)!
Everyone experiences personal problems from time to time, but sometimes they can become overwhelming. Most employers know that associates who are distracted by personal problems cannot do their best on or off the job. Work life as well as family life can suffer. By providing an Employee Assistance Program (EAP), an employer can encourage associates and their families to seek help, at no cost, when a problem first appears and can be most easily resolved.
Your privacy is a critical part of the EAP's success. Because confidentiality is such a vital concern, no information about any case will ever be reported to your employer without your written consent.
Eligible people can call the Matrix EAP with any kind of personal problem, such as:
- Family or marital problems
- Stress or anxiety
- Financial or legal concerns
- Substance abuse
- Anything else that concerns you or your family

EAP Materials Order Form
We strongly encourage our corporate clients to promote the Matrix EAP to their associates and family members using a variety of our pre-printed materials available at no extra cost.
Supervisor Referral of Employee(s) to EAP
If you would like to formally refer to the Matrix EAP an employee whose job performance is suffering, we ask that you and the employee(s) download and complete the following forms and fax them to Matrix at 614-475-9821 prior to the employee making any appointments with us.
This form will assist the supervisor in describing the specific areas in which an employee's performance is problematic. Both the supervisor and the employee should sign the form.
Authorization to Obtain/Release Protected Information
This form permits Matrix to communicate with the supervisor regarding information relevant to the employee's case. Both the supervisor and the employee need to sign this form if the supervisor would like information about the case. Without this form, NO information can be shared with the supervisor regarding the case due to privacy laws.
Legal & Financial Assistance
When a situation involves legal or financial concerns, sometimes employees need help sorting things out. Whether they have been in a car accident, have trouble managing their monthly bills, are facing divorce or have concerns about estate planning, advice from a lawyer or financial planner can be extremely beneficial.
As part of the Matrix EAP, employees have access to free legal and financial telephonic advice and discounted legal office referrals for themselves and their dependents. The service can assist with:
- Family law
- Consumer problems
- Credit problems
- Housing
- Criminal problems
- Accidents and injuries
- Debt and bankruptcy
- Wills
- Estate planning
- Traffic violations and more
A simple call will put employees and their families in touch with attorneys and financial experts who will provide free telephonic advice or a free legal office consultation. A 25% discount is provided for ongoing legal representation. This benefit is entirely confidential, and employees cannot use this service for legal action against their employer.
Employees may also visit our website to access a comprehensive legal library of information, downloadable legal documents such as living wills and powers of attorney, free personal will preparation, and a glossary of legal terms.

Work & Family Benefits Program Information
With a Matrix EAP, associates and their families have access to a number of work/life benefits via our web partner, My Life Values. My Life Values offers employees easy, online access to a proprietary database of over one million work-life providers, useful and compelling research and information, robust concierge resources, online mortgage assistance, as well as shopping rewards and discounts at a network of national vendors.
Using My Life Values, employees can gather information and make important decisions about family, health, leisure time, and how to get "the most for their money" all through one web tool they can use at their own pace and when their schedule permits.
Workplace Crisis Stabilization
The Matrix workplace crisis stabilization program provides rapid access to professional crisis intervention services for all human resources, security, and supervisory associates. Matrix will guide associates through the traumatic time after a workplace crisis to minimize its effects by:
- Creating structure in the midst of the crisis
- Offering a safe haven for associates and others
- Making order out of chaos
Matrix EAP clients pay no fee for materials, on-site orientations, telephone consultations or the first four hours of an on-site workplace intervention. Workplace on-site situations that could merit a Matrix response include:
- Robbery or attempted robbery
- Death (suicide, murder, accidental or unexpected)
- Kidnapping or hostage situations
- Significant injury or illness
- Unusual and disturbing customer interaction
- Accidental fire or arson
- Illegal activity such as embezzlement or theft
- Sudden and unexpected removal of someone from a job position
- Natural disasters
EAP Supervisor Training
We strongly encourage all supervisors to take advantage of the Supervisor Training availableby clicking in this section ofour website. The training will:
- Make you more comfortable with the process of constructively confronting a troubled employee.
- Help you identify situations where a referral is appropriate
- Demonstrate the difference between a formal referral to the EAP, where the employee signs an authorization for the psychologist to communicate with the manager/Human Resources, and a more casual referral, where you might want to suggest the EAP but do not need to have any feedback.
There will be some questions and answers at the end of each section. A certificate will be issued at the end of the instruction, which takesapproximately 30-45 minutes to complete
